“Dr. C” Conchitina Crisostomo, age 47, Jonesborough, passed away Monday, January 21, 2019 at Johnson City Medical Center.
Dr. C was born in Manila, Philippines and the daughter of Diana Chua Crisostomo, Manila and the late Carlos Palanca Crisostomo.
She was a Nephrologist with East Tennessee Medical Associates.
Survivors in addition to her mother, include her husband, Dr. Joseph Lee, Jonesborough; son, Alexander Lee, Jonesborough; brother, Clifford Joseph Crisostomo, Manila; three sisters, Caroline Lourdes Crisostomo, Buena Park, CA, Candice Rosario Balmaceda, Fremont, CA and Consuelo Teresa Crisostomo, Manila; father & mother-in-law, Chung H. & Kuei Y. Lee, Bethlehem, PA; sister-in-law, Jennifer Kazemi (Shahin Kazemi), Cherry Hill, NJ.
Memorial Mass to celebrate the life of Dr C (Conchitina Crisostomo) will be on Friday, January 25, 2019 at 10 am at St. Mary's Catholic Parish 2211 E Lakeview Dr. Johnson City, TN 37601. Rosary prayer at 9:30 am prior to the mass.

I am so very sorry to hear my beloved Dr. C has passed away.
I was a patient of hers, and she was the most loving, caring
doctor a patient could have. I will always remember her
beautiful smile as she walked in the room, and made me feel I
was important to her. Sending my condolences to her
husband, son, and family that God will watch over you during
this difficult time. Heaven gained an angel.

both became acquainted with Dr C. There. She will always be
remembered by us as a very skilled doctor, who was always
kind and approachable. A blessing to humanity and to her

Mrs. Doyle Johnson
My husband was a patient of Dr. C both at JCMC
and Fresenius. She was an excellent doctor and
a very caring compassionate person who cared
not only for her patients but for their
families as well. She always remembered to ask
my husband about my health problems. He passed
away last March and we were both saddened to
hear of her health problems which led to her
being on leave. He would be very sad to hear
of her passing but glad to know that she is now
pain-free as is he. My deepest sympathy to her
family.
I am deeply saddened to hear of the death of my cousin Chi-Chi. I wish I could be there with the family to grieve and to celebrate Chi-Chi's life. I will always remember the fun times we had together. My condolences to Joe and Alex, and the Crisostomos.

I worked with Dr. C at the VA several years ago. She was always so friendly and approachable. I just wanted to let you know how sorry I was to hear of her passing.
I’m so very sorry. Dr C was my first nephrologist after I started

fondly remember the joy she displayed at the birth of her son.
